Martin Svende claims that the reason is a bug that was previously known. The new version was released on August 24 and all users were encouraged to install the update.
One of the Ethereum developers reports that Flexpool, BTC.com and Binance were running on the old version, which caused the consensus error.
This error was exploited by cybercriminals, which caused the reorganization. This bug affects some EVM-compatible chains, for example, Polygon, Binance Smart Chain (BSC), xDAI, Görli.
Another serious bug was fixed in November last year, but that time they did not focus on it, so as not to attract the attention of potential scammers.
This approach led many operators to ignore the update, which then led to the largest outage on the Ethereum network. The accusations fell on the developers, because they should have drawn the attention of the platforms to the criticality of the update.
